Requests for Dose Records

Records for Individuals Monitored

To request a dose history report (NRC Form 4) for an individual monitored at an NRC licensed facility (10 CFR 20.2206), or for an NRC employee, or for monitoring records for a DOE facility, you may use the Automated Dose History Request Form or go to the How to Submit a Dose History Request page for further instructions.

Release Waiver for Privacy Information

Requests must be accompanied by a release waiver (organizational, individual) signed by the monitored individual. Records can be searched by name and date of birth, but it is recommended that the request include the individual's social security number in order to ensure the accuracy of the retrieval. Requests that include the individual's social security number need to be protected in accordance with the Privacy Act of 1974 and should be marked as 'Controlled Unclassified Information (CUI)'.

Requesting Dose Records
Outside of REIRS and REMS

Air Force records are maintained at the Air Force Dosimetry Center at Wright-Patterson Air Force Base, Ohio. Army records are maintained at the Army Dosimetry Center located at Redstone Arsenal, Alabama. Navy, Marine Corps, and Naval Reactor records are maintained at the Naval Dosimetry Center at Bethesda, Maryland. These three dosimetry centers also provide occupational radiation monitoring services for AFRRI, DLA, DTRA, USUHS, the Coast Guard, and certain federal entities and consequently maintain associated records for these military and federal entities.
